You probably know all kinds of applications that contain lots of features, but at the cost of usability. With very few exceptions, most off-the shelf software packages sold these days could have actually benefited from being held at an earlier level of release. With PC software, the drive has always been to make it bigger and add more features, more functions, and more options. There are good business reasons for this. If a company isn t seen as having the latest, the biggest, the best, then they risk losing sales to the competition. Also, bringing out a new release once a year gives customers, both existing and new, the comfort factor that the software company still cares and is supposedly listening to the requirements of the customers. However, it has been estimated that no user of Microsoft Word, for example, ever uses more than ten percent of the total functionality of the program, and that s after extensive training.

A fact of some importance is that the left recurrent laryngeal nerve, because of its long course under the aortic arch, may be damaged as a result of thoracic disease. There is no dysphagia with lesions at this point in the nerve since the branches to the pharynx (but not to the larynx) have already been given off. For this reason, an aneurysm of the aortic arch, an enlarged left atrium, mediastinal lymph nodes from bronchial carcinoma, and a mediastinal or superior sulcus lung tumor are much more frequent causes of an isolated (left) vocal cord palsy than are intracranial diseases. It is estimated that in one-quarter to one-third of all cases of paralysis of the recurrent laryngeal nerve no cause can be established, i.e., they are idiopathic. Laryngeal neuralgia is a rare entity, in which paroxysms of pain are localized over the upper portion of the thyroid cartilage or hyoid bone on one or both sides. It is evoked by coughing, yawning, talking, or sneezing. In the case reported by Brownstone and coworkers it was relieved by carbamazepine.
